Make an informed decision

The Informed Consent Agreement outlines how the information you provide for each survey will be used and stored. It also talks about the risks and benefits of collecting health information so that you can decide whether you would like to join the Registry.   

Once you agree to the information in the Informed Consent Agreement, you can participate in the Registry and complete your first survey.
