Make an Informed Decision

In order to decide whether or not you wish to allow your data to be used for future research, you should know about the risks and benefits to make an informed decision. The Informed Consent Acknowledgment for each survey gives you information about  how your data will be used. Once you understand the data collection and sharing process, you will be asked if you wish to take part; if so, you just click the checkbox. If you have questions about any of the information presented on the Informed Consent Acknowledgment, contact registry@go2.org.

Once you agree to the Informed Consent Acknowledgement and the website terms and conditions, you can participate in the Registry and complete your first survey!
